Is the Youth Toastmasters Club's president one of the "youth" participants, or is it the adult sponsor who leads everything?

Just to clarify matters, there actually aren’t any Youth Toastmasters Clubs, within the Toastmasters organization.

The category of ‘Gavel Clubs’ is in place to provide structure and support for groups that are unable to pay the registration fees, are unable to attend due to being underage or unable to attend local meetings e.g. a prison-based club.

Gavel Clubs are able to use the same structure that regular Toastmasters clubs do including voting in officers and using the educational program. However, they are not able to submit their educational achievements to Toastmasters International for recognition.

Theoretically, an unemployed adult sponsor could be elected as President, but it would likely be more effective if it was one of the youth members that took the role on.
